First Graders must be 6 by September 1

Second Graders must be 7 by September 1

General Application Information:

·         Please read the information in this packet carefully. It contains an outline of Sacred Heart School’s Mission, Philosophy, Student Learning Expectations, programs, tuition, and fees.

·         Note that “tuition break status” requires a minimum $300 identifiable yearly contribution to the church collection using your church envelope or a check with your envelope number on it. In order to qualify, you must have been a member of our parish for at least one year and present the church contribution statement that was mailed to your family in January. If you do not qualify, you may choose to apply for probationary rate. Please see FAQ sheet for more information on probationary rate.

·         Fill out each form neatly and completely.

·         All applicants will be tested regardless of space available. Testing will be as follows:

·         Grades 1-8 in fall 2010, tested on Wed., March 24^th  3:15-4:00p.m.

·         Kindergarten in fall 2010, testing is scheduled on March 16^th and 17th (siblings only) and March 22^nd and 23^rd for new applicants and siblings.

·            Call the school office (626)332-7222 to schedule your kindergarten testing appointment.

·            If you miss your testing date, you will lose priority status in the admission process. After priority admission, further testing appointments will be made if spaces are available.

·            It is very important that you give the Confidential Recommendation Form to your child’s current teacher IMMEDIATELY. Your child’s suitability for admission cannot be evaluated without this form. The form is to be mailed directly

 (or faxed) to the school by the teacher and must be received by March 12, 2010 in order to ensure priority status in our registration process.

THE SCHOOL DOES NOT ACCEPT RECOMMENDATIONS THAT ARE BROUGHT IN BY THE PARENT.

·         All forms enclosed in this packet, filled out completely.

·         The original (and a copy for our files) of your child’s birth certificate.

·         The original (and a copy for our files) of your child’s Baptismal certificate

·         The original (and a copy for our files) of your child’s Communion certificate                (if child is entering grade 3-8)

·         A copy of the most recent report card and testing scores (as applicable)

·         Up-to-date immunization records for your child

***Please note that we will be unable to evaluate new students for admission until all required paperwork is received (including the recommendation form). Priority is given to supporting and active parishioners first.
